We believe in getting to know our patients on a personal level so that visits to our office feel more like they are visiting family. It is our practice to create friendships with our patients that last many years and to build their trust in us as their dental team.

We want our patients to experience superior dental care, and provide certain comforts that elevate office visits. When you visit our office, you can relax and watch television, sit by the fireplace, and enjoy our fish tank.

QC Dental Care Plan Coverage

First time primary members receive a free box of Opalescence Go Whitening Trays
Two regular cleanings per year covered at 100%
Two dental exams per year covered at 100%
One emergency exam and image as needed (during business hours) covered at 100%
One set of Bitewing radiographic images per year covered at 100%
One Panorex or Full Mouth series or images every 5 years covered at 100%

Two fluoride treatments per year for children through age 18 covered at 100%
One fluoride treatment per year for adults age 19 and up
20% Off Basic and Major restorative procedures
10% Off Implant restorative procedures (reminder that our office does NOT place implants)
Free – unlimited consultations

QC Dental Care Plan Limitations and Exclusions

Adult Membership for ages 19 and over, and Child Membership 18 and under.

Patient’s portion of their bill is due on the day of treatment.
Cannot be used in conjunction with another dental plan, automobile or medical insurance plan, or discount.
Cannot be used for services as a result of injuries, which are covered under Worker’s Compensation or Employer’s Liability Insurance.
Periodontics, endodontics, orthodontics, oral surgery, pediatrics, and any other services needed to be done by a doctor outside of our facility.
Any services that cannot be performed because of the general health, physical or psychological limitations of the patient.
Additional treatment for gum disease, known as scaling and root planning and periodontal maintenance is not part the initial exam and cleaning.
Yearly benefits do not roll over to the next year.
No refunds on premiums will be issued at any time if participant decides not to utilize the dental program.
Not transferable.
Use of Care Credit financing will reduce membership discounts by 5%.
There will be a $50.00 Broken or Missed Appointment Fee without 24 hour notice.
